Billing

We will bill you for our services separately from the surgeon and surgical facility.  The staff at our local business office are available to answer your questions.  They bill your primary and secondary insurance free of charge for you, based on the insurance information you provide to the surgical facility.

You are responsible for paying your bill within sixty days of billing.  All account balances over 60 days will be charged interest at a rate of .75% per month.

As of January 2010, we are "Participating" physicians with: Asante, Blue Cross, Champus,  ClearChoice, HealthNet, LifeWise, Medicare, ODS, PacificSource, and PHP health plans. We also participate in Jackson County's Medishare program and Oregon's Medicaid Program.
